Return-to-Duty SAP Evaluation

Professional Substance Abuse Professional (SAP) evaluations for DOT-regulated, safety-sensitive employees who are required to complete the return-to-duty process following a DOT drug or alcohol testing violation.

This evaluation includes a comprehensive review of the employee’s history, current circumstances, substance use concerns, and DOT requirements to determine whether appropriate education, treatment, and compliance recommendations are needed in accordance with federal DOT regulations (49 CFR Part 40).

Ohio SAP Services provides a respectful, supportive, and professional process focused on helping employees understand requirements, successfully navigate compliance expectations, and work toward safe return-to-duty eligibility.

Follow-Up SAP Evaluation
$100.00

The follow-up SAP evaluation is a required step after you complete any recommended treatment or education. Its purpose is to confirm you followed recommendations and to determine whether you are eligible to return to safety-sensitive duties.

What the follow-up evaluation includes

  • Review of compliance with recommendations

  • Determination of successful completion

  • Return-to-duty eligibility documentation

  • Follow-up testing plan recommendations to the employer/DER

What to expect at the appointment

  • A review of the documentation you provide and a discussion of your treatment progress

  • Review of your recovery plan

  • A written report from Ohio SAP Services outlining the determination and any testing recommendations

If we determine you have not completed recommended services, we will explain what remains to be done and provide additional recommendations and guidance on next steps.
