The follow-up SAP evaluation is a required step after you complete any recommended treatment or education. Its purpose is to confirm you followed recommendations and to determine whether you are eligible to return to safety-sensitive duties.

Determination of successful completion
Return-to-duty eligibility documentation
Follow-up testing plan recommendations to the employer/DER
What to expect at the appointment
A review of the documentation you provide and a discussion of your treatment progress
Review of your recovery plan
A written report from Ohio SAP Services outlining the determination and any testing recommendations
If we determine you have not completed recommended services, we will explain what remains to be done and provide additional recommendations and guidance on next steps.
